you will find a link to all Student Organization
Forms followed by a brief description of the
form. All forms are in PDF format. If you have
questions about a form or have difficulty opening
a file, please contact the Office of Student
Activities.
Fundraising
Approval Form
All campus fundraising projects must be approved
by the Office of Student Activities. This policy
includes all student organizations, athletic teams,
departmental clubs and honorary societies and
includes both on and off campus fundraising projects.
Please allow sufficient time for all appropriate
signatures.
CAN
Sound Equipment Request Form
Any recognized student organization may request
to use the CAN sound equipment. Organizations
interested in using the sound equipment need to
submit a Sound Equipment Request Form two weeks
prior to the event. Organizations interested in
using the new sound equipment must arrange for
a trained CAN member to be present at the event.
Organizations submitting a request less than two
weeks in advance will be charged a $20 late fee
for the old equipment or a $30 late fee for the
new equipment.
Application
for Office Space in the Student Organization Center
All organizations interested in having an office
in the new Student Organization Center are encouraged
to fill out an application. Currently all offices
are occupied. Organizations interested in being
placed on a waiting list are encouraged to submit
an application to the Office of Student Activities
at any time. Be sure to read the Student
Organization Center Policies before applying.
Intention
to Organize a New Organization
The Intention to Organize a New Organization
is for students wishing to start a new organization
on campus. After completing this form and submitting
a proposed constitution, the new organization
will be asked to appear before the College Environment
Committee for final approval.
Student
Organization Recognition
All recognized student organizations are required
to submit the Student Organization Recognition
form to the Office of Student Activities by
October 1 each fall. The form provides important
contact information and updates to the Office
of Student Activities.
Officer
Update
All recognized student organizations are required
to notify the Office of Student Activities of
any changes to elected officers listed on the
Student Organization Recognition form.
President
Agreement
All presidents of recognized student organizations
are required to sign the President Agreement
which outlines the relationship between the
president and the Office of Student Activities.
The President Agreement is to be submitted with
the Student Organization Recognition form each
fall or when a new president is elected.
Advisor
Agreement
All recognized student organizations are required
to have a faculty or staff advisor. The Advisor
Agreement outlines the relationship between
the advisor, the student organization, and the
Office of Student Activities.
