If you participated in spring physicals given here at the college you must also make sure that the following forms are on file at Goldsmith Health Center. When you arrive on campus you will complete an update form to document any changes in your health since the previous physical. If you had any medical procedures performed (orthopedic surgery or any other surgery in general) over the summer you must bring documentation from that physician with information and clearance to participate in athletics regarding the procedure. If you did not participate in spring physicals then you must follow the same instructions given to the new and transfer student athletes.
