All new and transfer athletes need to have all of the paperwork below filled out and completed BEFORE your arrival onto campus. You must make arrangements to have a yearly physical evaluation preformed by a primary care physician of your choice using our medical forms. The following forms need to be completed and on file at Goldsmith Health Center before you can participate in any sort of team event.

o Pre-Participation Health History
o Physical Exam (Needs to be filled out by M.D.)
o Adrian College Athletic Release
o Health Insurance Information
