Q:
Do I have to complete a medical physical before
I participate in the Adrian College athletic program?
A: You must complete a Pre-participation Physical
Exam Form medical physical before participating
in any of Adrian College’s intercollegiate
athletic programs. A physical form can be obtained
from the athletic training room, Goldsmith Health
Center, or by clicking on the link above. We offer
mass athletic physicals every April for returning
athletes and new or transfer athletes must get
a pre-participation physical before coming onto
campus in the fall.
Q:
What forms do I need to fill out before arriving
at Adrian College as an athlete?
A: All athletes must complete a Pre-Participation
Health History, and Physical Exam form signed
by a physician. Also you must make sure that your
Insurance Information Form is also completely
filled out, along with signed Adrian College Athletic
Release. All of which must be on file at Goldsmith
Health Center before you as a student athlete
will be cleared for participation in Adrian College
intercollegiate athletics.
Q:
Do the forms need to be mailed earlier than arrival
at camp time?
A: Yes. All forms must be received by August 10th
for all fall sport athletes. Returning student
athletes may mail them back to the athletic training
department, and new and transfer student athletes
should mail them to Goldsmith Health Center.
Q:
Do I have to have health insurance?
A: It is required by Adrian College that all full
time students (12 credit hours or more) must have
health insurance. Whether it is your parents/guardian
group insurance or insurance through Adrian College.
If do not want the College’s insurance then
you must fill out a decline waiver form or your
student account will be charged for the services.

Q:
What if my parents/guardian do not carry health
insurance on me?
A: All full time students will be automatically
enrolled in the student health insurance available
through Adrian College. For further information
on the student health insurance please direct
your questions to Goldsmith Health Center (517)-265-5161

Q:
If I receive a medical bill from an injury I obtained
while competing for Adrian College athletics,
who do I contact?
A: Bring or mail any medical bills relating to
an athletic injury to Goldsmith Health Center
as soon as possible.
Q:
What if I get ill, such as with the flu, or have
a non-athletic injury while competing at Adrian
College? Who do I see for care?
A: Always contact the athletic training staff
first with any injury or illness. We will refer
you to either Goldsmith Health Center or proper
the physician for treatment of your injuries.
Q:
What are the hours of operation of the Athletic
Training Room?
A: The Athletic Training Room is open Monday –
Friday 9 am until noon for treatment, and also
1pm - 3pm. If a student athlete can not make into
the athletic training room for treatment due to
class conflicts then separate arrangements can
be made between the individual ATC and the student
athlete to receive treatment.
